At the conclusion of the survey, you will have an opportunity to opt-in to the draw for a chance to win a 6 month membership credit.

  Eligibility:

  The competition is open to individuals who have completed at least one of the six centre surveys in its entirety and opt-in to enter the draw.

  Employees of the centre and their immediate family members are not eligible to participate.

  Entry:

  Entry into the competition is subject to opting-in to the draw at the completion of the main survey.

  No purchase or payment of any kind is necessary to enter or win.

  Entries from each of the six centre surveys will be consolidated into one (1) major prize pool.

  Prize:

  One (1) major prize will be awarded, consisting of a 6-month membership credit applied to the winner's account at the centre.

  If the winner is an existing member of the centre, a 6-month membership credit will be applied to their account.

  If the winner is not currently a member of the centre, they will receive a 6-month free membership, which will conclude at the end of the 6-month period.

  In either case, the prize is non-transferable and may not be exchanged for cash or any other item.

  Winner Selection:

  The winner will be selected randomly from all eligible entries received.

  The draw will be conducted within 7 days of the survey's conclusion.

  The winner will be notified via email or phone within 14 days of the draw date.

  If the winner does not respond within 7 days of notification, an alternate winner may be selected.

  Prize Claim:

  To claim the prize, the winner must provide valid identification and any other information requested by the centre.

  The prize must be claimed within 30 days of notification.

  General Conditions:

  The centre reserves the right to modify or cancel the competition at any time, for any reason.

  By participating, participants agree to abide by these terms and conditions and the decisions of the centre, which shall be final and binding.

  The competition is subject to all applicable laws and regulations.

  By opting into the draw, participants indicate their acceptance of these terms and conditions.
